Fall 2015 DPT Admission Cycle
Fall 2015 DPT Admission Application Instructions -
Two (2) Separate Applications are required:
1) DPT Departmental Application via PTCAS
Application Available July 2, 2014 via PTCAS
Firm Deadline: October 15, 2014
NOTE: It is recommended that applicants applying for the DPT program meet with Admissions Coordinator, Crystal Bishop, periodically throughout the application process.
2) CSU Graduate Application - Fall 2015
Available through www.csumentor.edu
Deadline: November 30, 2014
Submit all official transcripts from each college and/or university attended to PTCAS and Fresno State by indicated application deadline dates (2 sets of all transcripts; 1 set to PTCAS and 1 set to Fresno State).
Verification of Observation Hours Form
It is REQUIRED to submit a PT Observation Hours form verifying your observation hours for every location and number of hours observed. Login to PTCAS and use the online PT Hours signature process or print each customized PT Observation Hours form, and have your hours verified by a licensed physical therapist. If using the paper verification option, mail the signed form directly to PTCAS. Please read all directions set by PTCAS to avoid any delays in the submission of your application.
The verification of observation hours form is ONLY for students who are not yet ready to apply this Fall. Click HERE for form.
Mail/Hand-deliver completed form directly to the PT Department at Fresno State. DO NOT mail to PTCAS.
Deadline: October 15, 2014
Arrange for three (3) letters of recommendation to be sent to PTCAS on your behalf; one/two each from a licensed physical therapist, and a non-Physical Therapy professor. Fresno State pre-physical therapy/IHRS faculty are not acceptable references. Only original letters will be accepted and must be in a sealed envelope.
