Key and Access Issuance

Requesting the Key
In order to obtain university keys, a key request must be processed and approved by authorized personnel. Log into your my.fresnostate.edu profile and click on “forms portfolio” on the on the left side of the screen. It will be listed under “Key Request Form”.
The form will automatically generate the individual’s information once you enter their employee or student ID number. Select metal key for the traditional physical key, or electronic for any door accessed by a card. The form provides a list of buildings and rooms from which the selection must be made. The form will automatically send to the appropriate authorizer before Lockshop personnel process the form.
For submasters, masters and any key requested for a student, a “purpose” section must be filled out at the bottom of the electronic form.
Both the individual who has submitted the key form, and the individual receiving the key will automatically receive notifications when the key is ready for pickup.
Keys are issued at the University Student Union Information Center. This is located on the main floor.
Note: When the campus is on break or the semester ends, key issuing will be located in the Reservation Center on the third floor of the University Student Union, not on the first floor Information Center.
For individuals wishing to return campus keys, this action can be completed at the University Student Union unless a separation form is required. Key Control staff must sign the separation form upon return of all state keys. University Student Union staff do not have authorization to sign this form. For separation form signature, please call 559.278.2172 or email Rachel Freeman at email@example.com to make an appointment at the campus Lockshop.
A request for a replacement key is completed on the same electronic form, but the “replacement” box must be checked to indicate that the individual requires lost or stolen keys to be replaced.
